Instructions
Using one of the Chart of Work documents above, list each major outcome of your project. An outcome is a specific change that occurs in individuals, groups, communities, organizations, or systems as a result of your project.
Beneath each outcome, list planned activities associated with that outcome. Activities are the processes, tools, events, technology, and actions that will lead to achieving your outcome. Indicate the duration of each activity on the timeline by marking an “X” in the relevant months. Also indicate the amount of funding requested from the GLFT to achieve the outcome, as well as the total project cost.
For each outcome, indicate the deliverable(s) associated with the outcome. Deliverables are the tangible results of your work toward each outcome. They can be written products, but this is not required. They may be processes, agreements, or organizational or physical structures, as well as reports, communications documents, and other literature.
On pages 1 and 2 of the chart there is space for ten outcomes and their associated activities and deliverables, for a period of 24 months. If your project will take more than 24 months to complete, please use pages 3–4 to chart work for up to 48 months. If you have more than ten major outcomes, use your “copy and paste” function to add more rows.
